These are the appointments available to returning patients. The new-patient evaluation is not shown because the booking system reserves it for new patients only.

This practice is private pay. We do not bill insurance, and you are responsible for payment for all services. We can provide a statement you can send to your insurance for reimbursement.

Names and doses if you have them, including anything from another prescriber.
